The two holes need to be lined up vertically. You then need to drop the cables from the top hole and fish them out from the bottom hole. It is also a good idea to put junction boxes inside the holes and use a coverplate to make things look better.
The biggest mistake people make is mounting the TV too high on the wall. It should be at a height so that you can look directly at it without looking up too much when you are seated. Above a fireplace is way too high.
Mounting a flat screen on the wall does look nice but just putting it on a TV stand is much easier.
