If you need to run cables behind the wall the basic way you do this is to cut the hole behind the TV near where the AV connectors are. You also cut a hole directly below that hole behind your equipment cabinet. Both of these holes should be in a spot that they will be hidden by the TV and equipment cabinet when you are done.

The biggest mistake people make is mounting the TV too high on the wall. It should be at a height so that you can look directly at it without looking up too much when you are seated. Above a fireplace is way too high.
